Frontal Fibrosing Alopecia London. The Receding Hairline That Is Often Missed

Frontal fibrosing alopecia, or FFA, is a scarring hair loss that causes the front and sides of the hairline to slowly recede, very often with loss of the eyebrows. It mainly affects women around and after the menopause, its incidence has risen sharply over the last two decades, and it is frequently mistaken for ordinary hairline thinning. The most important fact is that early diagnosis and treatment can halt it, while delay allows permanent loss.

This guide is written by the medical team at The London PRP Clinic by The Wellness, a doctor-led clinic across Marylebone and Canary Wharf. Scarring alopecias demand accurate diagnosis and honesty about what the evidence supports. Getting you assessed properly and early is the single most valuable thing we can do for FFA.

What is frontal fibrosing alopecia

FFA is a primary scarring alopecia, considered a variant of lichen planopilaris, in which inflammation gradually destroys the follicles along the frontal and temporal hairline and replaces them with scar tissue. The hallmark is a band of hairline recession that pushes the forehead back, often with a pale, scarred margin and small bumps or redness around the follicles. Both terminal and fine vellus hairs are lost in the affected band.

A very telling feature is eyebrow loss, which occurs in a large majority of cases, with studies reporting partial or total eyebrow involvement in around 63 to 83 percent of patients, and eyelashes can be affected too. FFA mainly affects postmenopausal women, though premenopausal cases are increasing, and the condition has grown so much since it was first described in 1994 that dermatologists describe its rise as an epidemic. The exact cause is not fully understood, with genetic, immune, hormonal and environmental factors all thought to contribute.

Can frontal fibrosing alopecia be reversed

It cannot usually be reversed, but it can be stopped, which is why early action matters so much. Because FFA is a scarring process, once a follicle has been destroyed and replaced by scar tissue, hair will not regrow from it. For this reason the goal of treatment is to halt the inflammation and stabilise the disease, preserving the hairline and follicles that remain rather than regrowing what has already gone.

This makes early, accurate diagnosis the decisive factor. The recession is gradual and easily dismissed as normal thinning or a high forehead, so many women lose treatable ground before the condition is identified. The practical message is simple. If your hairline is moving back, especially if your eyebrows are also thinning, do not wait. The earlier the inflammation is controlled, the more of your hairline you keep. Stabilisation achieved early is a far better outcome than treatment started once the band has widened.

How is frontal fibrosing alopecia treated

Treatment aims to switch off the inflammation driving the disease, and the options with the strongest evidence are anti-inflammatory and hormonal. The best-supported treatments are 5-alpha-reductase inhibitors such as finasteride or dutasteride, intralesional corticosteroid injections, and hydroxychloroquine. A 2024 randomised trial found finasteride and hydroxychloroquine equally effective at improving severity scores over six months, with more than 60 percent of patients improving. Topical steroids, topical calcineurin inhibitors, minoxidil and other agents are also used in combination.

The honest position on PRP matters here. PRP is not an established treatment for the scarring process of FFA itself, and any responsible clinic will say so plainly rather than market it as a fix. Where PRP can have a role is in supporting a coexisting androgenetic component, which is common, or general scalp health, alongside the proven anti-inflammatory treatment that does the real work in FFA. We will always be clear about which part of your hair loss each treatment is actually addressing.

Why early, doctor-led diagnosis matters most

FFA is regularly confused with a receding hairline from genetic loss, with menopausal thinning, or simply overlooked, and getting the diagnosis right determines whether the disease is stopped in time. At The London PRP Clinic by The Wellness, our GMC-registered doctors assess the hairline and eyebrows with dermoscopy, arrange a scalp biopsy where it is needed to confirm the diagnosis, and put in place evidence-based treatment to stabilise the condition.

We also test for the factors that affect any hair loss. Our doctors arrange blood tests covering ferritin and iron stores, full thyroid function, vitamin D, B12, key hormones, HbA1c and zinc, run from the same Marylebone clinic. This combination of accurate diagnosis, honest expectation-setting and correction of what is treatable is what separates a doctor-led clinic from a product seller. With a scarring alopecia, where lost hair does not come back, that rigour is the whole point.

How much does assessment and treatment cost in London

For a scarring alopecia, care should begin with a proper doctor-led assessment rather than a product, and that is where the real value lies. At The London PRP Clinic by The Wellness, you are seen and assessed by a GMC-registered doctor who sets out the diagnostic steps and an evidence-based plan to stabilise the condition, coordinating the anti-inflammatory treatment that FFA needs.

For reference, where PRP is appropriate for a coexisting androgenetic component, doctor-performed PRP for hair starts from £545 per session, against the £600 to £850 or more per session common at Harley Street and Mayfair flagship clinics, with your plan and any costs confirmed at consultation. Given that FFA causes permanent loss when left untreated, the value of an accurate, early diagnosis is hard to overstate.

Frequently asked questions about frontal fibrosing alopecia

What is the first sign of frontal fibrosing alopecia?

Usually a slowly receding frontal and temporal hairline, often with a pale scarred margin and redness or small bumps around the follicles, and frequently thinning or loss of the eyebrows. Because it is gradual, it is easily mistaken for normal thinning.

Can FFA hair loss grow back?

Hair does not regrow from follicles that have already scarred, so treatment aims to stop the inflammation and preserve the hairline and follicles that remain. This is why early diagnosis is so important.

Does PRP treat frontal fibrosing alopecia?

PRP is not an established treatment for the scarring process of FFA itself. The proven treatments are anti-inflammatory and hormonal. PRP may support a coexisting androgenetic component where present, alongside the main treatment.

Who gets frontal fibrosing alopecia?

It mainly affects women around and after the menopause, though premenopausal cases are rising. Eyebrow loss is common, affecting the majority of patients.

How is FFA diagnosed?

Through clinical assessment including dermoscopy and, where needed, a scalp biopsy to confirm the diagnosis, because FFA is easily confused with a genetic receding hairline or menopausal thinning.
