What Is a Pelvic Ultrasound?
A pelvic ultrasound is an imaging investigation that uses high-frequency sound waves to produce real-time pictures of the structures inside the female pelvis. These include the uterus, ovaries, fallopian tubes (when distended), endometrium (uterine lining), cervix, and bladder. It is non-invasive, painless and uses no radiation.
Two types of pelvic ultrasound exist. Transabdominal ultrasound is performed by moving a probe across the lower abdomen with a full bladder. Transvaginal ultrasound is performed using a slim probe inserted into the vagina, giving higher-resolution images of the uterus and ovaries because the probe sits closer to the structures being examined. Both are commonly performed in a single appointment for comprehensive assessment.
When Is a Pelvic Ultrasound Needed?
A pelvic ultrasound is indicated for a wide range of gynaecological and pelvic concerns.
Abnormal menstrual bleeding. Heavy, prolonged, irregular or post-menopausal bleeding.
Pelvic pain. Persistent or recurrent pelvic or lower abdominal pain.
Fertility investigation. Antral follicle count assessment, ovarian reserve estimation, structural causes of subfertility.
Suspected fibroids, ovarian cysts or endometriosis.
Investigation of polycystic ovary syndrome (PCOS).
Suspected pelvic mass on examination.
Surveillance of known gynaecological conditions.
Pre-IVF or pre-fertility-treatment workup.
Routine women's health screening. As part of a Women's Body Scan or executive screening package.
What Conditions Does a Pelvic Ultrasound Detect?
A consultant-led pelvic ultrasound is the standard imaging investigation for the following.
Uterine fibroids, including their size, number, location and impact on the endometrium. Endometrial pathology, including polyps, thickening (potentially significant in post-menopausal women), and structural abnormalities. Ovarian cysts, distinguishing simple from complex morphology and identifying features that warrant follow-up imaging or referral. Polycystic ovary morphology, identifying the characteristic ovarian appearance seen in PCOS. Adenomyosis features. Hydrosalpinx (fluid-filled fallopian tubes). Pelvic masses, with characterisation of likely origin. Bladder abnormalities visible from the pelvic approach. In fertility settings, antral follicle count (a key marker of ovarian reserve) and endometrial assessment for receptivity.
Findings on pelvic ultrasound directly inform clinical management. A large fibroid affecting the endometrial cavity may warrant hysteroscopy or myomectomy. A complex ovarian cyst may require gynaecology-oncology assessment. A low antral follicle count is a key data point for fertility planning.

How Much Does a Pelvic Ultrasound Cost in London?
A pelvic ultrasound starts from £795 at The Online GP by The Wellness for a single-region consultant-led scan. The Women's Body Scan is £2,495 and combines pelvic, bilateral breast and thyroid ultrasound. The Fertility Scan Package is £995 and combines pelvic ultrasound with antral follicle count and endometrial assessment.
Across central London, a private pelvic ultrasound typically costs between £250 at high-street sonographer-led imaging chains and £950 plus at consultant-led Harley Street clinics. The £795 standalone price at The Online GP by The Wellness reflects doctor-performed delivery with same-day interpretation rather than sonographer-performed with separate consultant report turned around days later.
What Is the Difference Between Pelvic Ultrasound and a Smear Test?
A cervical smear (cervical screening) collects cells from the surface of the cervix to screen for high-risk human papillomavirus (HPV) and pre-cancerous cellular changes. A pelvic ultrasound is an imaging investigation of the deeper pelvic structures including uterus and ovaries.
The two tests are complementary. A smear screens for cervical cancer risk at the cellular level. A pelvic ultrasound assesses structural pelvic anatomy. Most women aged 25 to 64 should have NHS cervical screening at recommended intervals (every 3 or 5 years depending on age and HPV status). A pelvic ultrasound is investigated when symptoms arise, fertility is being planned, or as part of broader women's health screening.
What Happens on the Day?
You are greeted privately and taken into a consulting room. The doctor reviews symptoms and history. For transabdominal ultrasound, you will be asked to have a full bladder (drink 500ml of water 30 to 60 minutes before the appointment). For transvaginal ultrasound, you will be asked to empty your bladder before the scan. You undress from the waist down and are covered with a sheet. The scan takes 15 to 30 minutes.
Findings are explained immediately. A written report is provided. If part of a broader package (Women's Body Scan or Fertility Scan Package), the additional scans are performed in the same visit. Blood results, where included, return within 24 to 48 hours with a follow-up consultation.

Is the Scan Safe and Are There Any Risks?
A pelvic ultrasound, whether transabdominal or transvaginal, is completely safe. It uses no radiation and no contrast injection. The only sensation is mild pressure from the probe. Transvaginal ultrasound is well tolerated by the vast majority of women, though it can be declined in favour of transabdominal-only imaging if preferred. There are no contraindications. Pregnant women can also undergo pelvic ultrasound.
A female chaperone is available on request and is offered by default for transvaginal examinations performed by male doctors. Patient comfort and dignity are central to how we run every appointment.
When Should I Choose the Women's Body Scan vs the Fertility Scan Package vs Standalone Pelvic Ultrasound?
Choose Standalone Pelvic Ultrasound from £795 when you have a specific pelvic concern (pain, bleeding, suspected fibroid or cyst, follow-up of a known finding) and no requirement for broader imaging.
Choose Fertility Scan Package at £995 when you are planning conception, investigating subfertility, or want a focused assessment of ovarian reserve and uterine receptivity. Includes pelvic ultrasound, antral follicle count and endometrial assessment.
Choose Women's Body Scan at £2,495 when you want comprehensive women's health screening covering pelvic, bilateral breast and thyroid ultrasound in one visit. This is the appropriate annual screening tier for adult women over 40 or earlier with family history.

Frequently Asked Questions
How much does a pelvic ultrasound cost?
From £795 at The Online GP by The Wellness for a single-region consultant-led scan. Women's Body Scan £2,495 (pelvic plus bilateral breast plus thyroid). Fertility Scan Package £995 (pelvic with antral follicle count and endometrial assessment).
What is the difference between transabdominal and transvaginal ultrasound?
Transabdominal ultrasound is performed across the lower abdomen with a full bladder. Transvaginal ultrasound uses a slim probe in the vagina for higher-resolution imaging of uterus and ovaries. Both are commonly performed in the same appointment for comprehensive assessment.
Is a pelvic ultrasound painful?
No. The scan is non-invasive and painless. Mild pressure may be felt during transabdominal imaging when a full bladder is required, and slight pressure during transvaginal imaging. Most women find both well tolerated.
Do I need a referral for a private pelvic ultrasound?
No. The Online GP by The Wellness is fully private. No NHS GP referral or registration required.
How long does the appointment take?
The scan itself takes 15 to 30 minutes. The full appointment including history, examination and explanation of findings typically takes 45 to 60 minutes.
When during my cycle should I have a pelvic ultrasound?
For most general indications, any time in the cycle is acceptable. For specific fertility assessments including antral follicle count, the scan is typically performed in the early follicular phase (cycle days 2 to 5). We will advise the optimal timing when booking based on the indication.
What if the scan finds something abnormal?
Most findings are benign or require only monitoring. Where significant, the doctor explains findings, places them in clinical context and arranges onward referral to a consultant gynaecologist or specialist as appropriate. We have established private referral pathways.
Can I have a pelvic ultrasound during pregnancy?
Yes, pelvic ultrasound is safe in pregnancy. Specific pregnancy ultrasound for fetal assessment is a separate investigation. Contact us to discuss the appropriate scan for your situation.
