Which route applies, and in what order

Start with your centre. Schools and colleges hold the relationship with the examination board, submit special consideration requests and initiate reviews of marking, and almost nothing at examination board level can be started by a candidate alone. Ring them the day you have your results rather than the week after, because centre deadlines for reviews and appeals sit inside the board deadlines and fill quickly.

In parallel, contact the university admissions office directly if a place has been declined or downgraded. Admissions teams have discretion, they use it, and a documented medical explanation for underperformance in a specific paper is the kind of thing they can act on while places remain. This route closes as courses fill rather than on a fixed date, which is why speed matters more here than anywhere else.

Then consider the longer options. A deferred place for entry in 2027 where the condition is ongoing. A reduced or reconsidered offer supported by evidence. Resitting with the illness documented so that the resit is judged fairly. Each of those decisions is easier for an institution to make when a doctor has set out what happened and when.

What the letter needs to establish

Four elements, and the third is the one that changes outcomes. The condition, stated clinically. The dates, matched precisely against the examination timetable, because a letter covering the wrong week is worse than no letter. The functional impact, meaning a reasoned clinical account of how the condition affected preparation, concentration, attendance or performance in the specific papers concerned. Where the illness was ongoing across a revision period rather than a single day, that should be described as such, because the strongest cases are frequently about weeks of impaired preparation rather than one bad morning. Where a condition is continuing, a sentence on prognosis helps an admissions team weigh a deferral against a reduced offer.

A doctor can write retrospectively, recording that you presented describing symptoms over a preceding period and giving a clinical opinion on whether that account is consistent with the condition found.
