What Does an Echocardiogram Actually Assess?

A transthoracic echocardiogram (TTE) is a detailed ultrasound of the heart performed through the chest wall. It provides a real-time, moving picture of your heart and assesses:

Heart chambers: The four chambers of the heart (left ventricle, right ventricle, left atrium, right atrium) are measured for size. Enlarged chambers can indicate heart failure, valve disease, or chronic high blood pressure.

Left ventricular ejection fraction (LVEF): This is arguably the single most important measurement from an echocardiogram. LVEF tells you what percentage of blood the left ventricle pumps out with each heartbeat. A normal LVEF is typically 55–70%. Reduced LVEF indicates impaired heart function and is a key criterion for diagnosing heart failure.

Heart valves: All four valves (mitral, aortic, tricuspid, and pulmonary) are assessed for stenosis (narrowing), regurgitation (leaking), and structural abnormalities. Valve disease is common — aortic stenosis alone affects approximately 3% of adults over 75 in the UK — and echocardiography is the definitive investigation for diagnosis and severity grading.

Wall motion: The walls of the heart are assessed during contraction. Regional wall motion abnormalities — where one section of the heart wall does not contract normally — can indicate previous heart attack (myocardial infarction) or reduced blood supply to that area (ischaemia).

Diastolic function: How well the heart relaxes and fills between beats. Diastolic dysfunction is increasingly recognised as a cause of heart failure symptoms (heart failure with preserved ejection fraction, HFpEF) and is only detectable on echocardiography.

Pericardium: The sac surrounding the heart is checked for pericardial effusion (fluid around the heart), which can occur after viral infections, autoimmune conditions, or as a sign of other systemic disease.

Great vessels: The proximal aorta and pulmonary artery are assessed where visible.


Who Should Get an Echocardiogram?

NICE clinical guideline CG95 recommends echocardiography as the first investigation for suspected structural heart disease. Research published in peer-reviewed cardiology literature shows that echocardiography identifies structural heart abnormalities in approximately 1 in 12 symptomatic adults who present to primary care — a significant detection rate.

Echocardiogram vs. Other Heart Tests: What You Need to Know

This is one of the most common areas of confusion for patients. Here is what each cardiac investigation does and when it is appropriate:

Echocardiogram (what The Wellness offers): Assesses heart structure, chamber size, valve function, ejection fraction, wall motion, and diastolic function. It does NOT image the coronary arteries.

ECG (electrocardiogram): Records the electrical activity of the heart. Detects arrhythmias, conduction abnormalities, and signs of previous heart attack. An ECG complements an echo but provides different information.

CT coronary angiogram (CTCA): Uses CT imaging with contrast dye to visualise the coronary arteries and detect blockages (atherosclerosis). This is the appropriate investigation if you are concerned about coronary artery disease specifically.

What to Expect During Your Echocardiogram

No special preparation is needed. You do not need to fast. You can eat, drink, and take your regular medications normally.

You will be asked to undress from the waist up and lie on your left side on an examination bed. Small adhesive ECG electrodes may be placed on your chest to monitor your heart rhythm during the scan. The doctor applies ultrasound gel and places the probe in several positions on your chest wall to obtain different views of the heart.

The scan takes approximately 30–45 minutes for a comprehensive TTE and 20–30 minutes for a cardiac screening echo. You may hear a whooshing sound when the doctor uses Doppler mode to assess blood flow through the valves — this is normal.

  • Replace a stress test for exercise-induced symptoms

Is an echocardiogram painful? No. It is completely painless. You may feel mild pressure when the probe is pressed against your chest.

Do I need to do anything to prepare? No fasting or special preparation is needed. Continue all regular medications.

Can an echocardiogram detect a heart attack? An echo can detect wall motion abnormalities that suggest a previous heart attack, but it cannot diagnose an acute heart attack in progress. If you are having chest pain now, call 999.

How often should I have an echocardiogram? This depends on your clinical situation. For general screening with no known heart disease, a single comprehensive echo provides a baseline.
