This is a significant diagnostic advantage because many shoulder conditions are only apparent during movement. A static MRI image cannot replicate this.

What a Shoulder Ultrasound Examines

A comprehensive shoulder ultrasound systematically assesses every soft tissue structure around the shoulder joint:

Rotator cuff tendons. The four rotator cuff muscles and their tendons are the most important structures assessed. The supraspinatus (the most commonly injured rotator cuff tendon), infraspinatus, subscapularis, and teres minor are each examined for tears (full-thickness and partial-thickness), tendinopathy (degeneration), and calcific deposits. Peer-reviewed research has demonstrated that ultrasound sensitivity for full-thickness rotator cuff tears is approximately 92–95%, comparable to MRI.

Long head of biceps tendon. Examined within the bicipital groove for tendinopathy, subluxation (partial displacement), dislocation, or tears. Biceps tendon pathology is a common cause of anterior shoulder pain.

Subacromial-subdeltoid bursa. The bursa is a fluid-filled cushion between the rotator cuff and the acromion. Bursitis (inflammation and thickening of the bursa) is one of the most common causes of shoulder pain and is clearly visible on ultrasound.

Acromioclavicular (AC) joint. The joint between the collarbone and the acromion is assessed for osteoarthritis, effusion, and osteophyte formation. AC joint pathology causes pain on the top of the shoulder, particularly when reaching across the body.

Glenohumeral joint. The main shoulder joint is checked for effusion (fluid) that might indicate inflammation, infection, or arthritis.

Dynamic assessment. This is ultrasound's unique advantage over MRI. Your doctor can watch the rotator cuff tendons glide beneath the acromion as you raise your arm, assessing for impingement — where the tendon is pinched between the humeral head and the acromion during movement. This real-time assessment cannot be replicated by any other imaging modality.


Common Shoulder Conditions Ultrasound Can Diagnose

Rotator cuff tear. Tears range from small partial-thickness tears (affecting part of the tendon) to large full-thickness tears (going all the way through). Symptoms include pain on overhead movements, weakness when lifting, and night pain. Ultrasound accurately identifies tear size, location, and degree of retraction — all critical factors for treatment planning.

Rotator cuff tendinopathy. Degenerative change within the tendon without a discrete tear. The tendon appears thickened, swollen, or structurally abnormal on ultrasound. This is the most common cause of shoulder pain in adults over 40.

Subacromial impingement syndrome. The rotator cuff tendons are compressed between the humeral head and the acromion during overhead movements. Ultrasound with dynamic assessment demonstrates impingement in real time.

Calcific tendonitis. Calcium deposits within the rotator cuff tendons that cause intense, sometimes sudden, shoulder pain. Ultrasound identifies these deposits clearly and can guide treatment (barbotage — ultrasound-guided aspiration and lavage of the calcium deposit).

Frozen shoulder (adhesive capsulitis). While ultrasound findings in frozen shoulder can be subtle, thickening of the coracohumeral ligament and joint capsule can support the clinical diagnosis.

Bursitis. Inflammation of the subacromial bursa, causing pain with overhead activities. Clearly visible on ultrasound as bursal thickening or fluid.

AC joint arthritis. Osteoarthritis of the acromioclavicular joint, causing localised pain on top of the shoulder. Ultrasound shows joint effusion, capsular hypertrophy, and osteophytes.

Biceps tendon pathology. Tendinopathy, tears, or subluxation/dislocation of the long head of biceps tendon.

Post-surgical assessment. Following rotator cuff repair or other shoulder surgery, ultrasound can assess healing, identify re-tears, and guide rehabilitation decisions.


Shoulder Ultrasound vs. MRI: Which Do You Need?

This is one of the most common questions patients ask. Both are excellent investigations — the answer depends on your clinical question.

Choose ultrasound when: Your primary concern is the rotator cuff, biceps tendon, bursa, or soft tissue structures. You want dynamic assessment (impingement testing). You want immediate results. You are claustrophobic. You want a cost-effective investigation. You need comparison with the opposite shoulder.

Choose MRI when: Your doctor suspects a labral tear (SLAP lesion), bone pathology (fracture, bone marrow oedema), cartilage damage, or intra-articular loose bodies. MRI provides superior assessment of these structures.

Both may be needed: In some cases, ultrasound provides the initial assessment and MRI is arranged if further detail is required.

What to Expect During Your Shoulder Ultrasound

No preparation is needed. Wear a top that can be easily removed or pulled down from the shoulder — a vest top or T-shirt works well.

You will sit on a chair or examination stool. The doctor applies warm gel and examines each structure of the shoulder systematically. You will be asked to move your arm into different positions — resting by your side, hand behind your back, arm across your body — to assess different tendons and perform dynamic impingement testing.

The scan takes approximately 20–30 minutes. Both shoulders can be assessed in the same appointment if needed (useful for comparison).

Your doctor explains findings during the scan, shows you the relevant structures on the monitor, and discusses treatment options immediately afterwards. A written report is issued the same day.


Treatment Options After Your Shoulder Scan

Based on your scan findings, your doctor may recommend:

Physiotherapy. The first-line treatment for most shoulder conditions including tendinopathy, mild impingement, bursitis, and small tears. Your doctor can provide specific guidance on what your physiotherapist should focus on based on your scan findings.

Ultrasound-guided injection. For significant bursitis, AC joint arthritis, or calcific tendonitis, an ultrasound-guided corticosteroid injection can provide rapid relief. Unlike blind injections (without imaging guidance), ultrasound-guided injections are placed precisely into the target structure, improving accuracy and effectiveness. These can be performed at The Wellness in the same or a subsequent appointment.

Barbotage. For calcific tendonitis, ultrasound-guided barbotage involves aspirating and lavaging the calcium deposit under local anaesthetic. This can provide dramatic symptom relief.

Surgical referral. For large full-thickness rotator cuff tears, recurrent instability, or conditions not responding to conservative management, your doctor can arrange referral to an orthopaedic shoulder specialist.

Monitoring. Some findings (small partial tears, mild tendinopathy) may be managed with conservative treatment and monitoring, with a repeat scan to assess progress.

Frequently Asked Questions

Is a shoulder ultrasound painful? No. You may experience mild discomfort if your shoulder is already painful, particularly during positional testing, but the scan itself involves no pressure or injection.

Can ultrasound detect a frozen shoulder? Ultrasound findings in adhesive capsulitis can be subtle, but coracohumeral ligament thickening and restricted dynamic movement support the clinical diagnosis. The primary value of ultrasound in suspected frozen shoulder is excluding other conditions (rotator cuff tear, bursitis) that may mimic frozen shoulder symptoms.

How soon after injury should I have a scan? For acute injuries, scanning within 1–2 weeks allows swelling to settle while still providing early diagnosis. For chronic or gradual-onset shoulder pain, scan at any time.

Can both shoulders be scanned? Yes. Bilateral shoulder assessment is possible in the same appointment and is useful for comparison.
