What a Thyroid Ultrasound Examines
A comprehensive thyroid ultrasound systematically assesses:
Thyroid gland size and morphology. The entire gland is measured and its overall texture evaluated. A diffusely enlarged gland (goitre) may indicate Graves' disease, Hashimoto's thyroiditis, iodine deficiency, or multinodular change. A heterogeneous (uneven) texture can suggest chronic thyroiditis.
Thyroid nodules. Every nodule is measured in three dimensions and characterised by its composition (solid, cystic, or mixed), echogenicity (how bright or dark it appears compared to surrounding tissue), margins (smooth, irregular, or ill-defined), calcifications (microcalcifications are associated with higher malignancy risk), shape (taller-than-wide orientation is a suspicious feature), and vascularity on Doppler (blood flow pattern). These features are used to assign a TI-RADS score.
Cervical lymph nodes. The lymph nodes in the neck are assessed for size, shape, and morphology. Abnormal-appearing lymph nodes in the context of a thyroid nodule can change the clinical significance and management pathway.
Surrounding structures. The carotid arteries, jugular veins, and strap muscles are incidentally visible and assessed for any abnormalities.
TI-RADS: How Thyroid Nodules Are Classified
The American College of Radiology TI-RADS (Thyroid Imaging Reporting and Data System) is a standardised scoring system used worldwide to classify thyroid nodules and determine which ones need biopsy. Your doctor at The Wellness will apply TI-RADS to any nodule identified:
TR1 — Benign: Features consistent with a benign nodule (e.g., pure cyst, classic colloid nodule). No FNA needed.
TR2 — Not suspicious: Very low risk features. No FNA needed regardless of size.
TR3 — Mildly suspicious: FNA recommended if the nodule is 2.5cm or larger. Follow-up if 1.5cm or larger.
TR4 — Moderately suspicious: FNA recommended if the nodule is 1.5cm or larger. Follow-up if 1cm or larger.
TR5 — Highly suspicious: Features strongly associated with malignancy. FNA recommended if the nodule is 1cm or larger. Follow-up if 0.5cm or larger.

When Should You Get a Thyroid Ultrasound?
You can feel a lump in your neck. Any palpable lump in the thyroid region should be investigated with ultrasound. Most will turn out to be benign nodules or cysts, but characterisation is essential.
Your doctor found a thyroid abnormality on examination. If your GP or another doctor has detected thyroid enlargement, asymmetry, or a nodule on physical examination, ultrasound is the next step.
Abnormal thyroid blood tests. Elevated or suppressed TSH, abnormal T3/T4, or elevated thyroid antibodies may prompt ultrasound to assess the gland's structure.
Visible neck swelling or goitre. Visible enlargement at the base of the neck warrants imaging to determine the cause.
Difficulty swallowing or voice changes. Large thyroid nodules or goitres can compress the oesophagus or recurrent laryngeal nerve. Ultrasound helps identify the cause.
Family history of thyroid cancer or thyroid disease. Patients with first-degree relatives who have had thyroid cancer may benefit from baseline thyroid ultrasound.
History of neck radiation. Previous radiation exposure to the neck or chest area increases thyroid cancer risk. Regular ultrasound surveillance may be recommended.
Incidental thyroid finding on another scan. Thyroid nodules are sometimes discovered incidentally on CT, MRI, or PET scans performed for other reasons. Dedicated thyroid ultrasound provides the detailed assessment needed to characterise these findings.
Monitoring known thyroid nodules. Patients with previously identified thyroid nodules may need interval surveillance scans to monitor for growth or change.

What to Expect During Your Thyroid Ultrasound
No preparation is needed. No fasting. No full bladder. You can eat, drink, and take your medications normally.
You will lie on your back on a comfortable examination bed with your neck slightly extended (a small pillow may be placed under your shoulders). The doctor applies warm gel to the front and sides of your neck and moves the ultrasound probe gently across the area.
The scan typically takes 15–20 minutes for a thyroid assessment and 20–30 minutes for a thyroid and neck combined scan.

Thyroid Nodules: What You Need to Know
Most thyroid nodules are benign. Thyroid cancer accounts for fewer than 5% of all thyroid nodules. The overwhelming majority are benign colloid nodules, cysts, or hyperplastic nodules that require no treatment.
Size alone does not determine whether a nodule is cancerous. A 3cm benign nodule and a 1cm cancerous nodule can coexist. Ultrasound characterisation — the features described above — is far more important than size alone in determining risk.
Not all nodules need biopsy. The TI-RADS system ensures that only nodules with suspicious features are referred for fine needle aspiration (FNA). This avoids unnecessary biopsies for clearly benign-appearing nodules.
Thyroid nodules can grow slowly. Surveillance scanning may be recommended for nodules that do not meet biopsy criteria, to ensure they remain stable over time.
Multinodular goitre is common. Many patients have multiple nodules, particularly women over 40. This is usually a benign condition but each nodule is assessed individually.
Thyroid Conditions Ultrasound Can Evaluate
Hashimoto's thyroiditis. The most common cause of hypothyroidism in the UK. Ultrasound shows a characteristic pattern of diffusely heterogeneous, hypoechoic thyroid tissue, often with reduced vascularity.
Graves' disease. The most common cause of hyperthyroidism. Ultrasound may show a diffusely enlarged gland with markedly increased blood flow on Doppler (sometimes called "thyroid inferno").
Multinodular goitre. Multiple nodules throughout the gland, often with mixed solid and cystic components. Each nodule is assessed individually.
Thyroid cysts. Pure cysts are almost always benign. Complex cysts (with solid components) require closer characterisation.
Subacute thyroiditis. Inflammation causing neck pain and thyroid tenderness. Ultrasound shows characteristic focal hypoechoic areas.
What Happens If a Suspicious Nodule Is Found?
If your ultrasound identifies a nodule with suspicious features (TI-RADS 4 or 5 above the size threshold), your doctor will explain the finding clearly and recommend fine needle aspiration (FNA) biopsy. FNA is a quick, minimally invasive procedure performed under ultrasound guidance — a thin needle is inserted into the nodule to collect cells for laboratory analysis.

How often should I have a thyroid ultrasound? This depends on your clinical situation. For surveillance of known nodules, your doctor will recommend an appropriate interval — typically 6–12 months for indeterminate nodules, or 12–24 months for low-risk nodules. For screening with no known abnormalities, a single baseline assessment is usually sufficient.
Can thyroid ultrasound diagnose hypothyroidism or hyperthyroidism? Ultrasound assesses the structure of the thyroid gland, while thyroid blood tests (TSH, T3, T4) assess its function.
