Which test do you actually need?
For most adults with suspected obstructive sleep apnoea, a home study is the right first test. NICE recommends home respiratory polygraphy first and polysomnography if that is negative but symptoms continue (NICE NG202). American guidance reserves the overnight study as a first test for people with significant heart or lung disease, nerve or muscle conditions, a history of stroke, regular opioid use or severe insomnia (AASM, 2017).

Why do sleep study prices vary so much?
An overnight study needs a sleep centre bed, a technologist awake all night and many more sensors to fit and score, so it costs more than a home recorder. Between clinics, the price can also depend on who reads the study. A study scored by a technologist and reported by a consultant takes more expert time than an automated report, and a results consultation adds a doctor's time again.
What should you ask any clinic before you pay?
- Who reports the study, and are they a consultant in sleep or respiratory medicine?
- Is a consultation to explain the result included, or charged separately?
- What happens if the home study fails or is inconclusive?
- Is treatment, such as CPAP or a mouth splint, included or arranged separately?
- Will you get a written plan you can take to another doctor?
What does it cost to do nothing?
Poor sleep has costs that are easy to miss. RAND Europe estimated that insufficient sleep costs the UK 1.36% to 1.86% of its economic output and about 207,000 working days a year, and that people sleeping under six hours have a 10% higher risk of death than those sleeping seven to nine (RAND Europe, 2016).
For sleep apnoea the stakes are higher. Untreated severe disease was linked with more than three times the rate of fatal heart attack or stroke in a ten-year study of men (Marin, 2005), and with a higher risk of road crashes (Tregear, 2009). Drivers can be fined up to £1,000 for not telling the DVLA about a condition that affects their driving (GOV.UK).
