What a full check-up costs in Milan
A full check-up in Milan costs €450 to €1,425 at San Raffaele's Punto RAF, and the most complete executive programmes at Humanitas, IEO and Centro Diagnostico Italiano carry no published price. Adding a total body MRI costs €1,200 at Centro Diagnostico Italiano and from €2,500 at Ospedale San Raffaele.

The highest published price
The highest check-up price we found on a Milan provider's own page is €1,425, the top of the band Punto RAF prints for its packages, which start at €450 [1]. The page gives the band without pricing each package, so ask which package sits where. Its brochure describes one all-inclusive fee, which stays the same whether or not you take every test the specialists suggest [2].
That fee covers more than the day itself. In the Check Up Personalizzato, second and third stage tests are included when they are done within 45 days, and the report arrives in about 12 working days for men and 17 for women [2]. A booking usually takes 10 to 20 working days to come round [2].
Punto RAF calls the Check Up Personalizzato its main package. Its base day starts with an internist visit, a full abdominal ultrasound, a cardiology visit with an ECG, an eye examination, an ear, nose and throat visit with a hearing test, a skin check, and blood and urine tests. A carotid ultrasound is added from 40. Men add a urology visit, and women add a gynaecology visit with a Pap test and, from 40, a mammogram [2].
The programmes sold on request
The most complete programmes in Milan are priced only on request. Humanitas San Pio X describes two HealthPro routes, Gold and Platinum, for companies and professionals, each with a medical tutor and a closing interview, and its page prints no price [3].
IEO runs its women's check-up in a single day that ends with a final examination by one of its doctors [4]. Centro Diagnostico Italiano runs its CDI-Check under a coordinating internist, with specialist visits, laboratory tests and imaging in the same day [5]. Neither page shows a price.
Auxologico's Personalizzato takes about 6 hours in a single day and has no published price either [6]. Humanitas also runs check-ups for women and for men under and over 45, each ending with an internist who goes over the results, and that page shows no price [7]. Ask each provider for a written quote with the full list of tests, and ask whether follow-up tests are part of the fee.
The ladder below the top
At the other end, Auxologico's Smart check-up costs €150. It pairs blood tests with one specialist visit, urology for men, or gynaecology with a Pap test for women [6]. It suits someone who wants a short yearly review and already has a doctor who follows them.
Between the two sit Auxologico's Classico, which takes about 4 hours and is set by sex and age band, and the Punto RAF packages, which take 3 to 6 hours [6] [1]. Punto RAF's Check Up Light is done at a second site in central Milan, with a version for men and one for women [1].
Adding a total body MRI
Some people add a scan to the check-up, and it is the largest single cost. Centro Diagnostico Italiano charges €1,200 for a total body MRI with two consultations with its doctors [8]. It says the scan is covered by agreements with the main supplementary health funds and private insurers, so check your own policy first [8].
If the scan leads to further private tests, Centro Diagnostico Italiano takes 20% to 50% off them, depending on the test [8]. Ospedale San Raffaele prices its Full Body Scan from €2,500, and offers a full blood check-up and specialist visits alongside it [9].
Results worth asking for by name
A Cochrane review found that general checks given to everyone did little to reduce deaths, so the case for a higher price rests on tests chosen for you [10]. These two results change what a doctor advises. Ask whether a package includes them before you compare prices, because two packages at the same price can hold quite different tests.
- An HbA1c shows your average blood sugar over recent months. In 11,092 adults, a level between 6.0% and 6.5% carried more than four times the later risk of diabetes of a level between 5.0% and 5.5% [11].
- A coronary calcium score is worth asking for when your age or family history gives a reason. In 6,722 adults without heart symptoms, a score above 300 carried almost ten times the risk of a coronary event of a score of zero [12].
