Shingles in 2026. Early Signs, Why the First 3 Days Matter, and Who Can Have the Vaccine

Shingles is a painful rash caused by the chickenpox virus reawakening years after the original infection, and the most important thing to know is that treatment works best when it starts early. It usually begins with tingling, burning or pain on one side of the body, followed a few days later by a band of blisters on the same side, most often on the chest or tummy, and sometimes on the face. Antiviral medicine is most effective when started within 3 days of the rash appearing, and it is particularly important for people over 50, people with a weakened immune system, severe pain or rash, and anyone with shingles near the eye. Shingles near the eye needs to be seen the same day, because it can damage sight. In England, pharmacists can treat uncomplicated shingles in adults under Pharmacy First, while a doctor is needed for shingles on the face, severe pain, a weakened immune system or other complications. The NHS shingles vaccine is now offered at 65 for people who turned 65 on or after 1 September 2023, to everyone aged 70 to 79, and to adults aged 18 and over with a severely weakened immune system. At The Wellness video consultations with photo review start from £150 and home visits cost £450. Enquire for in-person consultation and vaccination assessment, availability and fees. Vaccines and any separate tests are quoted before booking. Fees appear further down this page.

Key facts about shingles
- Shingles usually affects one side of the body, in a band or patch.
- Pain or tingling often starts a few days before the rash.
- Antivirals work best when started within 3 days of the rash appearing.
- Shingles on the face or near the eye needs same-day medical assessment.
- You cannot catch shingles, but someone who has never had chickenpox can catch chickenpox from the blisters.
- The NHS vaccine is offered at 65 or 70 depending on birthday, to everyone aged 70 to 79, and to adults with a severely weakened immune system.
What are the early signs of shingles
The first sign is often a tingling, burning, itching or painful feeling on one side of the body, sometimes with a headache or feeling generally unwell. A few days later a red rash appears in the same area, which turns into fluid-filled blisters, then dries and scabs over within 7 to 10 days.
The rash usually follows a band on one side of the chest or tummy, but it can appear anywhere, including the face, eye and ear. It does not cross the middle of the body.
Why the first 3 days matter
Antiviral medicines, such as aciclovir, valaciclovir or famciclovir, can reduce the severity and length of shingles and lower the risk of complications. They work best when started within 3 days of the rash appearing.
Treatment is particularly recommended for people over 50, people with a weakened immune system, those with moderate or severe pain or rash, and anyone with shingles affecting the face or eye.
Pain relief matters too. Simple painkillers may not be enough, and a doctor can prescribe stronger options where needed.

When is shingles urgent
Seek same-day medical help if shingles affects your face, particularly near the eye or on the tip of the nose, if you have eye pain, redness or blurred vision, or if you have a weakened immune system.
Seek same-day help for shingles near the ear with weakness of the face, hearing loss or dizziness, or if the rash is widespread rather than in one area.
Call 999 for confusion, a severe headache with a stiff neck, or difficulty breathing.
Is shingles contagious
You cannot catch shingles from someone with shingles. But the fluid in the blisters contains the virus, and someone who has never had chickenpox, or the chickenpox vaccine, can catch chickenpox from it.
Until the blisters have scabbed over, cover the rash, avoid sharing towels, and keep away from pregnant women who have not had chickenpox, newborn babies and people with a weakened immune system.
What is postherpetic neuralgia
Some people have nerve pain that continues after the rash has healed, called postherpetic neuralgia. It becomes more common with age and can last months. Early antiviral treatment and good pain control help, and specific nerve pain medicines can be prescribed if it develops. The vaccine also reduces the risk.
Who can have the shingles vaccine
The NHS offers the shingles vaccine as 2 doses. People who turned 65 on or after 1 September 2023 are eligible from 65. People who turned 65 before that date are eligible from 70. Everyone aged 70 to 79 is eligible. Adults aged 18 and over with a severely weakened immune system are eligible at any age.
If you are not eligible on the NHS, private vaccination can be arranged from age 50, after an individual assessment.
