Does the Vampire Facial Work. PRP With Microneedling for Acne Scars and Skin Texture

For acne scars, yes. A 2022 meta-analysis of 14 controlled studies and 472 patients found adding PRP to microneedling nearly tripled the odds of more than 50 percent improvement, with no increase in severe side effects. For glow and fine lines the evidence is thinner. The name is marketing. The procedure is medical and a doctor should perform it.

Key points

  • A 2022 meta-analysis in Frontiers in Medicine of 4 randomised and 10 split-face controlled studies, 472 patients in total, found microneedling with PRP gave an odds ratio of 2.97 for more than 50 percent improvement in acne scars on Goodman's scale, a significantly better mean score, and a satisfaction odds ratio of 4.15, with no significant increase in severe erythema or oedema.

  • A 2019 systematic review in the Journal of the American Academy of Dermatology of 13 studies found activated, leukocyte and platelet-rich plasma combined with fractional ablative laser over 2 or 3 sessions a month apart improved acne scarring.

  • A January 2026 split-face study in the Indian Dermatology Online Journal compared PRP with microneedling against microneedling alone for acne scars in skin types 3 to 6, extending the evidence to darker skin where post-inflammatory pigmentation is the main risk.

  • PRP for a medical purpose is treated by the MHRA as an unlicensed medicine that doctors may prepare for their own named patients, and the DHSC has confirmed PRP sits in the medium-risk amber tier of England's licensing scheme for non-surgical procedures.

If you are considering the treatment for scarring or texture, message us on WhatsApp or email team@thewellnesslondon.com and a doctor will tell you whether your skin is one the trials show responding.

What a vampire facial actually is

The name attached itself to the procedure after a celebrity photograph in 2013, and it has done the treatment no favours. What happens is ordinary regenerative dermatology. A small blood sample is drawn and spun to concentrate the platelets. The face is numbed with topical anaesthetic. A microneedling device, either a pen with fine needles or a stamp, creates thousands of controlled micro-channels in the skin, which on their own trigger collagen production and dermal remodelling. The PRP is applied to the skin during and after needling so that it penetrates the channels, and in some protocols it is also injected into scars directly.

Microneedling alone is an established treatment for atrophic acne scars. The question the trials answer is whether adding your own platelets to the needling improves the result, and for scars the answer is yes by a meaningful margin. For skin that is not scarred, and is being treated for texture, pores, dullness or fine lines, the evidence is weaker, and a clinic should say which of the 2 it is offering before it quotes a price.

What the evidence shows for acne scars

The 2022 meta-analysis in Frontiers in Medicine is the reference point. It pooled 4 randomised and 10 split-face controlled studies with 472 patients, all comparing microneedling with PRP against microneedling without it. Adding PRP produced an odds ratio of 2.97 for more than 50 percent improvement on Goodman's qualitative scar scale, meaning patients were nearly 3 times as likely to get a good result, with a significantly better mean scar score and a satisfaction odds ratio of 4.15. Severe redness and severe swelling were no more common with PRP than without. That is a clean finding across 14 studies with no heterogeneity on the main outcome.

The 2019 systematic review in the Journal of the American Academy of Dermatology reached a similar conclusion for a different combination, finding that activated, leukocyte and platelet-rich PRP with fractional ablative laser over 2 or 3 sessions improved acne scarring. The January 2026 split-face study in the Indian Dermatology Online Journal tested PRP with microneedling in skin types 3 to 6, where the risk of post-inflammatory pigmentation after any needling or laser is highest, which matters for a London population. Atrophic acne scars are one of the few aesthetic indications where PRP has a pooled analysis and a consistent direction.

Send us a photograph of your scarring on WhatsApp and a doctor will tell you which scar types you have and whether they are the ones that respond.

What the evidence shows for glow, pores and fine lines

This is where honesty separates a clinic from a menu. Most vampire facials sold in London are sold for general rejuvenation, and the evidence for that use is considerably thinner than for scars. There is no meta-analysis of PRP with microneedling for photoageing, fine lines or texture in unscarred skin. There are small studies reporting improvement, and a triple-blind split-face randomised trial comparing PRF with PRP for microneedling treatment of facial photoageing has been designed at Chang Gung Memorial Hospital but had not started recruiting when registered.

The mechanism is plausible. Microneedling on its own stimulates collagen, and platelets add growth factors to that process. Patients often report a brighter, smoother appearance for weeks afterwards, and that is a real effect of needling and of the temporary swelling that follows it. What has not been shown in a trial of adequate size is that adding PRP to microneedling gives unscarred skin a lasting improvement beyond needling alone. A doctor here would say that plainly, would treat scars with confidence, and would treat texture as a reasonable option with modest expectations rather than a proven one.

What the treatment involves and what to expect

A course rather than a session. Most protocols in the trials used 3 sessions about a month apart, and results build over 2 to 3 months as collagen remodels. Each session takes around 60 to 90 minutes including numbing. Afterwards the skin is red and feels sunburnt for 1 to 3 days, sometimes with pinpoint bleeding and mild swelling, and make-up should be avoided for 24 hours. Sun protection is essential for at least a fortnight, because needled skin pigments easily, and this matters most in darker skin types.

The pre-treatment rules are the same as for any PRP. Anti-inflammatory drugs interfere with platelet function, so ibuprofen, naproxen and similar are stopped for 2 weeks before. Active acne, cold sores, a skin infection, isotretinoin within the last 6 months, a history of keloid scarring, blood thinners and pregnancy are reasons a doctor may decline or delay. Depth matters. Needling for scars goes deeper than needling for texture, and depth is a medical decision about your skin, not a setting on a menu.

A scar self-check before any treatment

Not all acne scars respond to needling, and the type decides the treatment. This does not replace an examination.

  • Rolling scars, broad shallow depressions with sloping edges that improve when you stretch the skin. These respond best to microneedling with PRP.

  • Boxcar scars, wider depressions with sharp vertical edges. These respond partially, and deeper ones may need subcision or laser alongside.

  • Ice pick scars, narrow deep pits. These respond poorly to needling and usually need punch techniques or TCA cross, so a course sold for them is a course wasted.

  • Raised or keloid scars. Needling can make these worse, and they are a different treatment entirely.

  • Red or brown marks without a change in skin level. These are post-inflammatory marks, not scars, and usually fade with time and topical treatment rather than needling.

  • Active acne. Needling through active spots spreads bacteria and is a reason to treat the acne first.

Our doctors put it this way. For acne scars this is one of the best-evidenced things we do. For a glow it is a reasonable treatment with modest evidence, and the honest thing is to charge and promise accordingly.

What the vampire facial does not do

It does not remove ice pick scars, raised scars or deep static wrinkles. It does not add volume or lift tissue. For unscarred skin the evidence that PRP adds anything lasting to microneedling is thin, and any clinic presenting it as proven for glow is presenting scar evidence for a different purpose. It is a course, not a single session, and a single session sold at a discount is a demonstration. It carries a real risk of pigmentation in darker skin if depth and aftercare are wrong, and of infection if the environment is not clinical. It is not available on the NHS.

What it costs in London, and what a cheap one costs

Doctor-led PRP facial and under-eye work at leading London clinics runs £400 to £850 per session or course. PRP skin treatments are advertised in London from around £320 at the lower end of the market, and the vampire facial appears on the menus of salons and non-medical clinics at lower prices still.

The reframing is who is holding the needle. PRP is an unlicensed medicine that doctors may prepare for their own named patients, and the DHSC placed it in the amber tier of the licensing scheme, meaning oversight by a qualified healthcare professional. A vampire facial in a salon is a blood product prepared and needled into the face by someone the regulation was written to exclude. The price difference buys the absence of a doctor, and for a procedure that draws blood and opens the skin, that is the one thing worth paying for.

Care at The London PRP Clinic by The Wellness is doctor-led and priced below flagship rates, with your exact figure confirmed at the free suitability review once a doctor has assessed your scar type or your skin concern.

Ask about the cost for your skin on WhatsApp and we will explain what the review covers.

The free suitability review

Every enquiry here starts with a free, doctor-led suitability review rather than a booking. A GMC-registered doctor examines your skin, classifies any scarring, checks the medical exclusions, and gives one of two answers. Either you are approved for treatment, with a course length, a needling depth and realistic expectations that reflect whether you are being treated for scars or for texture, or you are not approved, with an honest explanation of what would serve you better, whether that is treating active acne first, a different scar technique, or nothing at all.

We decline a meaningful share of the people who enquire. That is deliberate. A clinic that approves everyone is optimising for bookings, not outcomes.

Frequently asked questions

Does a vampire facial work for acne scars

Yes. A 2022 meta-analysis of 14 controlled studies and 472 patients found adding PRP to microneedling nearly tripled the odds of more than 50 percent improvement in acne scars, with higher satisfaction and no increase in severe side effects. Rolling scars respond best, ice pick scars poorly.

Does a vampire facial work for glow and fine lines

The evidence is thin. There is no meta-analysis of PRP with microneedling for unscarred skin, only small studies. Microneedling alone stimulates collagen, and whether PRP adds a lasting benefit for texture or fine lines has not been shown in a trial of adequate size.

How many vampire facial sessions do you need

Most trials used 3 sessions about a month apart, with results building over 2 to 3 months as collagen remodels. A single session gives temporary brightness from the needling and swelling, not a lasting result.

Is a vampire facial painful

Topical anaesthetic is applied first, so the needling is uncomfortable rather than painful. The skin is red and feels sunburnt for 1 to 3 days afterwards, with pinpoint bleeding and mild swelling common. Deeper needling for scars is more uncomfortable than needling for texture.

Is a vampire facial safe for darker skin

With the right depth and strict sun protection, yes, and a 2026 split-face study tested it in skin types 3 to 6. The main risk is post-inflammatory pigmentation, which is why depth and aftercare should be set by a doctor rather than a menu.

Who should not have a vampire facial

Anyone with active acne, cold sores or a skin infection, isotretinoin within 6 months, a history of keloid scarring, a bleeding disorder or blood thinner use, or who is pregnant. Anti-inflammatory drugs should be stopped for 2 weeks beforehand.
