Testosterone Testing for Men. When to Test and What Results Mean
Testosterone is essential for energy, mood, libido, muscle mass, bone density and overall vitality. Levels decline at roughly 1 to 2 percent per year from age 30 (British Society for Sexual Medicine, 2024), but symptomatic deficiency can occur at any age. UK prevalence estimates suggest 2 to 6 percent of adult men have clinically low testosterone, rising sharply with age and metabolic disease.
Symptoms that warrant a testosterone test include persistent fatigue, reduced libido, erectile difficulties, loss of muscle mass or strength, increased abdominal fat, low mood, irritability, poor concentration and brain fog, reduced motivation, and sleep disturbance. Three or more symptoms together should prompt formal testing.
A reliable hormone panel measures total testosterone, free testosterone and SHBG (sex hormone-binding globulin), together with LH, FSH, oestradiol, prolactin, DHEA-S and cortisol for a complete endocrine picture. The sample must be drawn before 11am because testosterone follows a diurnal rhythm. Results require clinical interpretation alongside symptoms, history, weight, sleep, alcohol and training load.
PSA Testing and Prostate Health Screening
The PSA (Prostate Specific Antigen) blood test measures a protein produced by the prostate. Raised PSA can indicate prostate cancer but can also reflect benign prostate enlargement (BPH), prostatitis, recent ejaculation, vigorous cycling, or urinary tract infection. A raised PSA does not confirm cancer. A normal PSA does not exclude it.

Testicular cancer is the most common cancer in men aged 15 to 49 and one of the most treatable when caught early. Stage 1 testicular cancer carries a 5-year survival rate above 95 percent (Cancer Research UK, 2025). Any new lump, swelling, ache or change in shape or size of a testicle should be investigated within days, not weeks.

At What Age Should Men Have a Health Check?
A sensible baseline is age 30, followed by an annual or biennial review from 40 onwards.
For men in their 20s, the priority is monthly testicular self-examination (this decade carries the highest testicular cancer risk) and a Targeted Blood Panel from £295 if symptomatic.
For men aged 30 to 40, a Comprehensive Blood Panel from £495 including lipids, HbA1c, vitamin D, thyroid function and testosterone (if symptomatic) establishes a benchmark for future comparison.
