Minoxidil Shedding Phase. Why It Happens And When To Worry

Many people notice more hair falling out in the first few weeks after starting minoxidil, and many stop at exactly the wrong moment. The extra shedding usually means the treatment is working. Minoxidil pushes follicles out of their resting phase and into a new growth phase, and the old resting hairs are shed to make way. It typically appears within the first 2 to 8 weeks and settles within a few weeks. Stopping throws away the start you have just made.
Key points
- Shedding after starting minoxidil is common and is caused by resting hairs being released as follicles re-enter active growth.
- It usually appears within the first 2 to 8 weeks of treatment and settles within a few weeks.
- The hairs coming out were already due to shed. Minoxidil brings their timing forward rather than causing extra loss.
- Shedding that continues well past 3 months, or that comes with scalp symptoms or patchy loss, is not the expected pattern and should be assessed.

Why minoxidil makes you shed first
The hair cycle
Every follicle cycles through a long growing phase, a short transition, and a resting phase of around 100 days, after which the hair releases and a new one begins. At any time most follicles are growing and a smaller proportion are resting.
In pattern hair loss, the cycle is disrupted. Growing phases become shorter, so each hair grows for less time and comes through finer, and more follicles sit in the resting phase at once.
What minoxidil does to it
Minoxidil shortens the resting phase and pushes follicles into a new growth phase sooner, then keeps them growing for longer. When a resting follicle is pushed into growth, the new hair forming beneath the old one pushes the resting hair out.
So a group of hairs that would have shed gradually over the following weeks and months are released together, early. That is the shed. It is a sign the follicles are responding, and the hairs that replace them are the ones that give minoxidil its benefit.
What to expect, and when
Timing
The increase in shedding typically begins within the first 2 to 8 weeks of starting minoxidil, whether topical or oral, and usually settles within a few weeks. It can recur to a lesser degree after a dose increase, for the same reason.
What it looks like
Diffuse shedding across the areas being treated, often most noticeable on wash days and on the pillow. The hairs coming out are resting hairs, typically with a small white bulb at the root, and many of them are already thin and short, since those are the miniaturised hairs the treatment is replacing.
What comes next
As the shed settles, new growth begins. Visible improvement in density usually takes around 3 to 6 months, and the fullest effect takes up to a year. Judging minoxidil at 6 weeks, in the middle of the shed, is judging it at its worst moment.

Should you stop
Almost always no. Stopping in the middle of the shed means you experience the shedding and none of the regrowth that follows it. If you then restart later, you usually go through the shed again.
The exceptions are side effects rather than shedding. With topical minoxidil, persistent scalp irritation, redness or itching may need a different formulation. With oral minoxidil, lightheadedness, a racing heart, swelling of the ankles or around the eyes, or chest discomfort should be reported to your doctor promptly. In a study of 1,404 patients on low-dose oral minoxidil, lightheadedness occurred in 1.7%, fluid retention in 1.3% and a fast heart rate in 0.9%.
When shedding is not the minoxidil shed
The expected shed has a recognisable pattern and a natural end. Get it assessed if any of the following apply.
It continues well beyond 3 months after starting, or keeps getting heavier rather than settling.
It is patchy, with defined bald areas, rather than diffuse.
Your scalp is red, scaly, tender or burning, or you notice a smooth shiny band at the hairline. These suggest a scarring alopecia, where follicles are permanently destroyed, and that needs prompt assessment rather than more minoxidil.
The shedding started before the minoxidil, or began after an illness, an operation, childbirth, stopping the pill or rapid weight loss, which suggests telogen effluvium from a separate cause.
You have not had bloods checked. Low ferritin, thyroid dysfunction, low vitamin D and low zinc all cause shedding and blunt the response to minoxidil. A ferritin result just above the lower limit is frequently reported as normal while being too low to support good regrowth.
Getting more from minoxidil
Take monthly photographs of your parting, hairline and crown in the same light. The shed feels dramatic and regrowth is gradual, and photographs are the only reliable way to see the second happening.
Use it consistently. Minoxidil only works while it is being used, and missed days add up.
Correct any deficiency found on blood testing, since a follicle short of iron responds poorly to any treatment.
Consider combination treatment. Pooled trial data show PRP combined with minoxidil outperforms either alone, and PRP, which uses growth factors from your own blood, has no hormonal action. It is supported by a 2025 meta-analysis in Dermatology and Therapy pooling 43 randomised controlled trials and 1,877 participants which found it increases hair density.

What assessment and treatment cost
A treatment plan should make clear what you are paying for: assessment of the cause of shedding, review of your medication and a plan for follow-up. If PRP is being considered alongside minoxidil, ask why it is appropriate for your diagnosis and what improvement will be measured.
Our care starts with a doctor-led suitability review, with blood testing arranged where needed and any treatment plan and exact cost confirmed in writing before you decide. For most people reading this in their first 2 months of minoxidil, the honest advice is to keep going and review at 6 months.
The suitability review
Every enquiry begins with a free doctor-led suitability review rather than a booking. A GMC-registered doctor examines your scalp with dermoscopy to see whether the follicles are cycling normally, miniaturising or scarred, reviews or arranges your bloods, and gives one of two answers.
Approved for treatment, where combination therapy would strengthen your result. Or not approved, which here is often the answer, because what you are seeing is the expected minoxidil shed and the right plan is to continue. We decline a meaningful share of these enquiries on exactly that basis.

Frequently asked questions about minoxidil shedding
Why am I losing more hair after starting minoxidil?
Minoxidil pushes resting follicles into a new growth phase, and the resting hairs are shed as new ones form beneath them. It is a sign the follicles are responding.
How long does the minoxidil shed last?
It usually appears within the first 2 to 8 weeks of treatment and settles within a few weeks. Shedding continuing well beyond 3 months is not the expected pattern.
Should I stop minoxidil if I am shedding?
Almost always no. Stopping during the shed means you experience the shedding without the regrowth that follows, and restarting later usually triggers it again.
Does the minoxidil shed happen with oral minoxidil too?
Yes. The mechanism is the same whether minoxidil is applied to the scalp or taken as a tablet, and it can recur mildly after a dose increase.
When will I see results from minoxidil?
Visible improvement usually takes around 3 to 6 months, with the fullest effect up to a year. Judging it during the shed is judging it at its worst moment.
When should I worry about shedding on minoxidil?
If it continues well past 3 months, is patchy rather than diffuse, comes with scalp redness, scaling or tenderness, or started before the minoxidil or after an illness or other trigger.
