What executive health assessment is

An executive assessment is a planned check for people without symptoms, meant to find risks early enough to change them. The value lies in the few measurements that predict disease, such as blood pressure, cholesterol, blood sugar and fitness, and in acting on what they show.European Heart Journal ESC guideline, 2021

A large review of general health checks found they made little or no difference to deaths from any cause or from heart disease when offered to whole populations. A check is worth more when it is aimed at your own risks and someone follows it through.Cochrane Database of Systematic Reviews Krogsbøll, 2019

Signs and causes

High blood pressure, raised cholesterol and early type 2 diabetes usually cause no symptoms for years. Long working hours, travel, poor sleep and little exercise all push them the wrong way, which is often why people in demanding jobs book an assessment.European Heart Journal ESC guideline, 2021

Fitness predicts how long people live as strongly as any single risk factor. In a study of more than 120,000 adults who had a treadmill test, lower fitness was linked with higher death rates across every age group.JAMA Network Open Mandsager, 2018

Send us what you are seeing and we will tell you what it is likely to be.

How it is diagnosed

Your ten year risk of a heart attack or stroke is calculated from age, blood pressure, cholesterol, smoking, family history and other conditions. It turns separate numbers into one figure that shows whether treatment is worth starting. The BMJ Hippisley Cox, 2017

A single high blood pressure reading in the clinic is not a diagnosis. Where it is 140/90 or higher, the reading is confirmed with a home or 24 hour monitor before anything is decided.NICE NG136, 2023

An HbA1c of 48 mmol/mol or more points to type 2 diabetes, and a level just below it shows a raised risk that changes in diet and activity can often reverse.World Health Organization WHO/NMH/CHP/CPM/11.1, 2011

What can be done about it

  • A focused blood panel

Advanced bloods read by a doctor, covering the markers that predict heart and metabolic disease. Enough for many people who feel well and have no strong family history.European Heart Journal ESC guideline, 2021

  • A full day assessment

Bloods, ECG, body composition and a long consultation in one day, with the results explained before you go home. Suited to people short of time who want one doctor to see the whole picture.European Heart Journal ESC guideline, 2021

  • A longer programme

Imaging, fitness testing and repeat reviews through the year for people who want the plan held and followed up, not a single snapshot.JAMA Network Open Mandsager, 2018

What to expect

Most people leave with one to three things worth changing, often blood pressure, cholesterol, weight or activity. The UK target is 150 minutes of moderate activity a week plus muscle strengthening on two days, and most adults in demanding jobs fall short of it.UK Chief Medical Officers CMO report, 2019

An assessment cannot rule out every disease, and a normal result on the day is not a guarantee. Symptoms that appear later still need to be seen, whatever the last check showed.Cochrane Database of Systematic Reviews Krogsbøll, 2019

What you can do before you come

Write down any family history of heart attack, stroke, diabetes or cancer, with the age each relative was when it happened. Heart disease in a parent or sibling before 60 counts as a risk factor in its own right when your heart risk is worked out.The BMJ Hippisley Cox, 2017

Bring a list of your medicines and supplements, including doses. If you check your blood pressure at home, bring a week of readings, which are often more useful than a reading taken on the day.NICE NG136, 2023

What not to buy

A scan of the whole body is not part of the day and is not needed by most people who feel well. Unexpected findings are common, around 3 in every 100 brain scans in healthy adults, and most turn out to be harmless after further tests. The BMJ Morris, 2009

Longer lists of blood tests are not automatically better. Each extra marker adds a chance of a borderline result that leads to repeat tests and worry without changing what you do.Cochrane Database of Systematic Reviews Krogsbøll, 2019

Questions people ask

The full day assessment is £2,995. It includes the time with one physician, advanced bloods, an ECG, body composition and a written plan. The fee comes off a membership or longevity programme in full if you take one within 30 days.

A standard check is usually a blood panel and a short appointment. Here one physician spends the day with you, the tests are chosen around your own risks, and the results are explained before you go home rather than sent in a letter.

Plan for most of a working day. The tests and consultation run at an unhurried pace, and some blood results take a few hours, which is why the review with your doctor comes at the end of the day.

It can. Some people book through their employer or claim the cost back, so check your company policy first. Whoever pays, your medical results stay confidential between you and your doctor unless you choose to share them.

No. The day covers bloods, an ECG and body composition. Imaging such as MRI is part of the longevity programmes, and a doctor can arrange a scan separately where your history or examination suggests one is needed.

Your doctor explains the result the same day and what should happen next. That may be a repeat test, a change in treatment, or a referral to a consultant, with a letter setting out what has been found and why.

There is no fixed rule. Your doctor suggests a time based on your results and risks. People with raised blood pressure, cholesterol or blood sugar, or who are making changes to bring them down, usually need shorter reviews in between.
