The fees, from the highest down

The highest membership fee published in Manhattan is $24,000 a year, at Metro Medical Direct. Before anyone becomes a member the practice charges $6,000 for a structured evaluation over three visits, which cannot be refunded and carries no obligation to continue. Laboratory tests and imaging are billed separately by outside providers, and the yearly fee can be paid annually, twice a year or quarterly [1].

Sollis Health publishes two starting prices. Platinum starts from $12,000 a year, and the final price depends on factors that include age and the services chosen [2]. Signature starts from $4,000 a year, and house calls, advanced testing and some vitamin infusions cost extra [3].

MDVIP, a national network of affiliated primary care practices, says its individual membership generally costs $2,500 to $5,000 a year, depending on location [4].

What the difference in price buys

Most of the distance between the lowest and the highest fee is time spent in your home and how complex the care is. Metro Medical Direct's fee pays for scheduled and as needed house calls, telehealth, ongoing written records and assessments of a person's capacity to make decisions, for older adults with complex medical and psychiatric needs [1].

Sollis Platinum includes three house calls a year for each member, two guest passes, a welcome consultation and one screening test a year from a short list, such as a heart CT calcium scan for members over 40, where available [2]. Signature members pay for each house call they book [3].

Ask any practice for the full list of what the fee includes, in writing, before you pay.

  • What exactly does the fee include, and what is charged on top?
  • How many house calls a year are included?
  • How many patients does each doctor look after?
  • Are laboratory tests and imaging billed separately, and by whom?
  • Can I pay quarterly, and what happens if I leave partway through the year?

Paying by the visit instead

If you need a doctor only a few times a year, paying by the visit can cost less than any membership. DOMUS charges $475 for a house call of 45 to 60 minutes at your home, office or hotel in any of the five boroughs, and $125 for a video visit anywhere in New York State. It states that these prices were current as of July 2026 and that any extra charges are disclosed before the visit [5].

Four house calls a year at that price come to $1,900, below the lowest membership range quoted above [5] [4]. A membership is worth more to someone who wants a doctor who already knows them, answers late in the evening and coordinates their specialists.

What insurance and Medicare will and will not pay

The American Medical Association's ethics code asks doctors in retainer practices to keep charges for their extra services clearly separate from the care an insurer or other payer covers [6].

For Medicare patients the rule is stricter. In 2004 the Office of Inspector General warned that a doctor who has agreed to accept Medicare rates and asks a Medicare patient for added payment for covered services risks substantial penalties and exclusion from federal health programmes. The doctor may still charge for services Medicare does not cover, and for deductibles and coinsurance [7].

If you are on Medicare, ask the practice whether it participates in Medicare and which services the fee pays for that Medicare does not.

What the research says the fee buys

One of the larger studies of membership medicine followed 10,186 MDVIP members and matched non-members for up to three years after joining. Members used emergency rooms and urgent care less, and a growing share of them cost less to care for over time [8].

The study was sponsored by MDVIP, and people who choose a membership may differ from people who do not, so it cannot show that the membership caused the difference. A 2025 review of the wider literature describes the evidence on clinical outcomes as limited [9].

What our membership costs

Our founding membership in New York is from $22,000 a year. It will give you and your family one doctor who keeps your records, sees you on the day you ask and arranges any test or specialist you need.

Our same day doctor visit in New York costs from $640, for those who prefer to pay by the visit.

Questions

How much does a concierge doctor cost in New York?

Published fees run from $2,500 a year at the lower end of MDVIP's range to $24,000 a year at Metro Medical Direct in Manhattan, which also charges $6,000 for its first evaluation. Sollis Health starts from $4,000 and $12,000 a year.

How much is a house call in New York?

DOMUS charges $475 for a house call of 45 to 60 minutes by a board certified family physician, at a home, office or hotel in any of the five boroughs. A video visit costs $125.

Can a concierge doctor charge Medicare patients a fee?

A doctor who participates in Medicare may not ask a Medicare patient for extra payment for services Medicare already covers, as the Office of Inspector General warned in 2004. The doctor may still charge for services Medicare does not cover.

Is a concierge doctor worth the cost?

The answer depends on how often you need a doctor. Four house calls a year paid by the visit cost less than the lowest membership fee, while a membership gives you a doctor who knows your history. One study sponsored by MDVIP found fewer emergency visits among members, and the wider evidence on clinical outcomes is limited.
